CityBase and Chase Bank are partnering to provide exceptional payment processing services for CityBase clients to accept card and check payments. To get onboarded with Chase, please complete the following forms.

This form will confirm which payment channels you are onboarding, such as web payments, POS, and kiosk payments. This is also where you will upload your completed documents. You will be able to start and save a draft of your response and resume work on it before it is submitted, if needed. You will need to provide:
2. Payment Processing Instructions and Guidelines (PPIG) - Merchant Agreement
Your Account Executive will supply you with the proper version of the Payment Processing Instructions and Guidelines (PPIG) Agreement. Please sign this agreement in all indicated places on this form. This agreement confirms that Chase Merchant Services will provide payment processing services for you as the merchant.
3. American Express PPIG Amendment (OPTIONAL) - Amex Merchant Agreement [attached below]
If you would like to accept American Express cards, please complete this PPIG Amendment.
4. W-9 Taxpayer Identification [see sample below]
5. Bank Letter for your Depository Account
You will need to request this from your banking institution. This is the account where your funds will be deposited.
